Ability First Australia’s NSW Systems Coordinator Program (NSWSCP) is a state funded initiative from the Ministry of Health, NSW.
The program is designed to provide a case management service to people under the age of 65 living in the community who present with complex care needs and are ineligible for Commonwealth funded aged care services and the National Disability Insurance Scheme (NDIS).
Systems Coordinators (SC’s) are located across NSW to assist individuals to access available supports and services across the health, community, disability and housing sectors. Our experienced staff bring a wealth of expertise in complex case management and service navigation, and our operating model is individualised, person-centred and underpinned by trauma informed care principles.
