Ability First Australia’s YPIRAC Systems Coordinator Program is a Commonwealth funded initiative from the Department of Health, Disability & Ageing.
This Program has been operating since late 2021 and is designed to provide a case management service (via its national network of Systems Coordinators) to people under the age of 65 living who present with complex care needs and are ineligible for Commonwealth funded aged care services and the National Disability Insurance Scheme (NDIS).
Systems Coordinators (SC’s) are located across Australia. Teams are based in QLD, NSW, VIC, WA and SA, providing outreach to NT, ACT and TAS to assist individuals to access available supports and services across the health, community, disability, and housing sectors. Our experienced staff bring a wealth of expertise in complex case management and service navigation, and our operating model is individualised, person-centred and underpinned by trauma informed care principles.
